Q  How do I file a claim against the City of Burbank?

 

You need to obtain a Claim Form from the City Clerk's Office, 275 East Olive Avenue, clicking here or call (818) 238-5851 and a form can be mailed to you. If the claim involves a collision with a City vehicle, you will need to complete a supplemental claim form as well. The forms must then be returned to the City Clerk's Office. It will be date stamped, logged, and sent to the City's Risk Management Division where it will be processed, telephone
